We’ll need their name, date of birth, address, contact number, diagnosis, medication, support needs, and how you believe Sea Sanctuary might be able to help.
From there, our team will be able to decide if Sea Sanctuary is able to offer support and contact the person in question to arrange an informal assessment. If they’d like you to be present for the assessment, that’s possible too.
Following the assessment we’ll get in touch to let you know if we can offer a place – we can’t tell you immediately as placement discussions are always completed as a team as there will be a number of people involved in your Sea Sanctuary journey.
If you’ve got any questions with regards to referring yourself or the assessment process, give Anita a call on 01326 378919 or email email@example.com
